Mammograms at OneHope NEO - MetroHealth

Mammograms at OneHope NEO - MetroHealth will be at OneHope NEO on Thursday, April 30 with their mobile mammogram bus, offering on-site mammogram screenings. To be eligible for a screening patients must be:

  • 40 years old or older

  • Last mammogram was at least 11 months ago

  • Not currently having breast problems

  • Not pregnant or breast feeding

Everyone that would like to get screened will have to sign-up. This is not a free event; MetroHealth will be billing people's insurance. There is a form that MetroHealth needs to be filled out by each person that would like to schedule an appointment. Once the form is filled out, we will submit it to MetroHealth and they will then follow up directly with each person to assign a time slot.

To complete an application you will need:

  • A list of all household members with birth dates and social security numbers;

  • Proof of income for all household members 18 years of age and older for at least the previous 30 days;

  • Proof of citizenship or legal residence for household members;

  • Copies of recent utility bills; and

  • Disability verification (if applicable).
